1. About this policy and who we are
Your Insurance Agency ("we," "us," or "our") is a licensed independent insurance agent (a sole proprietor) based in Ohio (license [license #]). This policy explains what we collect through this website, and through the phone calls, text messages, and emails that follow when you contact us or ask us to contact you. It covers those phone calls too. By using this site or submitting our form, you agree to the practices described here.
2. Information you give us
When you complete our quote form, we collect the information you provide:
- Your name, email address, and phone number;
- Your state of residence and age range;
- The coverage amount you are interested in.
We do not ask for your Social Security number, your date of birth, or any health or medical information on this website.
3. Information we record about your consent
When you check the consent box and submit the form, we keep a record of it: the exact wording you agreed to, the date and time, technical details about your browser, and the IP address recorded by our form's service when you submit. We keep this because telemarketing laws require us to be able to prove the consent you gave before we call or text you.
4. Information collected automatically
Like most websites, we and our providers automatically collect limited technical data using cookies, pixels, and tags: your IP address, device and browser type, the pages you view, and the site or ad that referred you. See Section 7 for the specific tools.
5. How we use your information
- To respond to your request and discuss insurance options;
- To contact you by phone, text message, and email (see Section 10);
- To schedule appointments and, if you ask us to, obtain quotes and submit applications to insurance carriers;
- To measure and improve our advertising and website;
- To keep records and comply with our legal obligations.
6. Who we share it with
We share your information only as needed to run our business:
- CallTools (our customer-management (CRM) and phone system), which receives your information and which we use to call, text, and manage contact with you;
- Cloudflare, which hosts this website and processes your form submission before passing it to our CRM;
- Calendly (appointment scheduling), if you choose to book a time;
- Our call-tracking provider;
- Insurance carriers and our marketing organization, when you ask us to seek coverage for you;
- Google and Meta, for advertising measurement and remarketing (see Section 7);
- Authorities or others when required by law.
Our service providers are contractually required to keep your information confidential and may not use it for their own purposes. We do not sell your personal information for money. We do use the advertising tools in Section 7, and under some state laws that use can count as "sharing" for targeted advertising.
7. Advertising, analytics, and tracking technologies
This site uses the following tools. You can opt out using the links provided.
- Google Analytics 4, including its Advertising Features (such as remarketing and demographics reporting). Google Analytics uses first-party cookies or identifiers together with third-party cookies or identifiers to understand how visitors use the site.
- Google Ads conversion tracking and Enhanced Conversions. We share limited information with Google so it can measure ad performance on our behalf.
- Remarketing. We use your visit data to advertise online. Third-party vendors, including Google, show our ads across the Internet, and use cookies and device identifiers to serve those ads based on your past visits to this site.
- Meta (Facebook) Pixel. Third-party storage technologies collect and receive information from this site and elsewhere; Meta and similar third parties use that information to provide measurement services and to target and deliver ads.
- Call tracking. A phone number shown on the site may be dynamically assigned so we can attribute your call to the ad that brought you here.

8. Do Not Track and browser signals
Because there is not yet a common industry standard for how to respond to "Do Not Track" browser signals, our site does not currently change its behavior when it receives one. You can still opt out of ad tracking using the tools listed in Section 7.
9. Third parties collecting across sites
Through the tools in Section 7, third parties including Google and Meta may collect information about your online activity over time and across other websites when you use this site.
10. How we contact you, and how to stop it
If you consent, we contact you by phone call, text message, and email about insurance. Consent is not a condition of any purchase. You can opt out at any time: reply STOP to a text, use the unsubscribe link in an email, or ask us on any call. We honor opt-out requests and keep an internal do-not-contact list.
11. Call recording and monitoring
We may record and monitor inbound and outbound phone calls for quality, training, and compliance. Where a call involves someone in a state that requires it, we announce the recording at the start of the call.
12. How long we keep information
We keep the information you submit for as long as we need it for the purposes above. We keep records of the consent you gave us (the exact wording, the date and time, and technical details) for at least five years after our last contact with you, because federal and state telemarketing laws require us to be able to prove that consent. We may keep information longer where the law requires it or where we need it to resolve a dispute.
13. How we protect information
We use reasonable measures to protect your information, such as access controls and secure connections. No method of transmitting or storing information is completely secure.
14. Your choices and how to reach us
You may ask us to access, correct, or delete the personal information we hold about you. Contact us using the details in Section 18, and we will respond as required by applicable law. If you apply for insurance, Ohio insurance law also gives you rights to access and correct the information used in that process.
15. Ohio insurance information practices
When you apply for coverage, you will receive a separate Notice of Information Practices required by Ohio insurance law, from us or from the insurance carrier. That notice is separate from this website policy and describes how information is collected and used in connection with your application.
16. Children
This site and our services are intended for adults 18 and older. We do not knowingly collect information from anyone under 18.
17. Changes to this policy
We may update this policy from time to time. Changes take effect when posted, and the "Last updated" date above shows the current version. If we make a material change, we will update that date and post the revised policy here.
